- Prescribe 40% total dose as scheduled, 40% PRN
- Discuss with patient – make an agreement
- Add “Opioid Use Agreement Exists” to Problem List
- Fill in IPP Chart using smartphrase “. SCDOPIOIDUSEAGREEMENT”
